Job summary

Maximus is seeking a Registered Nurse, Clinical Assessor to join our team and conduct person-centered assessments in accordance with contract directives and company processes. You’ll evaluate individuals in their homes, group homes, and care facilities to determine Long Term Care needs and placement options.

This role requires commuting within and covering Somerset County, Maine, with on-site travel dominating the schedule.

Qualifications

• Minimum two years of post-graduate nursing experience.
• Current RN license in good standing.
• Ability to travel up to 50% per week within Somerset County, Maine.

Responsibilities

• Complete medical and behavioral health assessments using standardized tools and criteria per contract requirements.
• Conduct on-site, video, and telephonic assessments to determine needs, eligibility and level of care.
• Adhere to HIPAA and confidentiality policies and document assessment outcomes accurately.
• Travel to client sites to perform in-person assessments exceeding 50% of the workweek.
