Job summary

Maximus is seeking a Registered Nurse, Clinical Assessor to join our team in Somerset County, Maine. You will conduct person-centered assessments in homes, group homes, and care facilities to determine Long Term Care needs and appropriate placement options.

This role requires commuting within and covering Somerset County, Maine, with on-site, telephonic, or video assessments as program requirements dictate. A nursing license and experience in LTC or home health are preferred.

Qualifications

• Minimum two(2) years of post graduate nursing experience.

Responsibilities

• Complete medical/behavioral health assessments using standardized tools and criteria per contract requirements.
• Conduct on-site, video, and/or telephonic assessments to determine needs and level of care.
• Document assessment outcomes accurately following HIPAA and confidentiality policies.
• Travel to client locations to perform in-person assessments for more than 50% of the workweek.
