Job summary

Maximus is seeking an Registered Nurse (RN) Clinical Assessor for a hybrid remote role serving Suffolk County, MA. You will perform in-person assessments in members’ homes and other settings, develop care plans, educate and advocate for clients, and collaborate with care teams to determine LTSS eligibility.

The position requires an active MA RN license, at least two years of post-graduate clinical experience, and comfort traveling frequently.

Qualifications

• Active RN license in Massachusetts.
• Minimum of two years of post-graduate clinical experience.
• Experience with vulnerable populations (behavioral health, I/DD, LTSS).
• Experience conducting home or community assessments.
• Knowledge of MassHealth LTSS and HCBS programs.
• Proficient with Microsoft Office and documenting in EHR.

Responsibilities

• Complete medical and behavioral health assessments per contract requirements.
• Conduct on-site, telephonic or video assessments to determine needs and eligibility.
• Develop and manage service plans for complex members.
• Educate members, families and caregivers on available services.
• Travel to client locations for in-person assessments (50%+ of workweek).
• Maintain HIPAA compliance and accurate documentation.
