Job summary

Maximus is seeking a Registered Nurse, Clinical Assessor to join our team in a hybrid remote role serving Kennebec County, Maine. You will perform person-centered assessments in homes, group homes, and facilities to determine Long Term Care needs and placement options.

The role requires commuting within Kennebec County and may involve travel to multiple sites. You will conduct on-site, video, and telephonic assessments, document outcomes under HIPAA, and comply with all confidentiality policies.

Qualifications

• Active Registered Nurse license
• Two years post-graduate nursing experience preferred
• Ability to commute within Kennebec County and cover counties as needed

Responsibilities

• Complete medical/behavioral health assessments using standardized tools per contract requirements; escalate complex cases
• Conduct on-site, video, or telephonic assessments to determine needs, eligibility and level of care
• Document assessment outcomes accurately in HIPAA-compliant systems and follow confidentiality policies
• Travel to client locations, performing in-person assessments for >50% of the workweek to support program delivery
