Job summary

Enlyte is seeking a Field Case Manager in the Buffalo, NY area. This full-time, hybrid role involves coordinating care for injured workers, collaborating with physicians, employers, and patients to design effective treatment plans and return-to-work strategies.

You will manage cases, travel locally as needed, and support cost-effective healthcare delivery while maintaining high quality clinical standards and documentation across multiple stakeholders.

Qualifications

• Active RN license in good standing.
• 2+ years clinical practice preferred.
• URAC-recognized certification in case management preferred (CCM, CDMS, CRC, CRRN, COHN/COHN-S, RN-BC, ACM, CMAC, CMC).
• Must have reliable transportation and be able to travel to in-person appointments.
• Proficient with Excel, Word and Outlook Email.

Responsibilities

• Develop and implement treatment plans to return injured/disabled workers to work.
• Collaborate with treating physicians/providers and employers to ensure cost-effective care.
• Interview injured workers to assess progress and determine occupational vs non-occupational status.
• Work with employers and physicians to modify job duties for early return to work.
• Independently manage workload, prioritize cases and prepare status updates for customers.
