Job summary

Enlyte is seeking a Field Case Manager in the Rancho Cucamonga, CA area. This full-time hybrid role requires local travel to in-person patient appointments.

You will work with physicians, employers, and patients to develop and implement cost-effective treatment plans that maximize recovery and safe return to work. The ideal candidate is a licensed RN with 2+ years of clinical experience, strong advocacy, and excellent communication skills.

Qualifications

• Associates Degree or Bachelor’s Degree in Nursing or related field.
• 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Active RN license in good standing; URAC-recognized case management certification options.

Responsibilities

• Create and implement treatment plans to return injured workers to work safely.
• Collaborate with physicians and employers to tailor treatment plans.
• Schedule and attend in-person appointments and coordinate care across stakeholders.
• Manage workload and prioritize cases to ensure timely care and outcomes.
• Prepare status updates and reports for customers as needed.
