Job summary

Enlyte is seeking a Field Case Manager in the Jacksonville, FL area. This full-time, hybrid role requires regular local travel to in-person patient appointments. You will work with physicians, employers, and patients to create effective treatment plans and coordinate care for workers’ compensation cases.

The ideal candidate holds an RN license with 2+ years of clinical experience and has strong written communication and computer skills. Travel and reliable transportation are required.

Qualifications

• Associates or Bachelor’s degree in nursing or related field.
• 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Active RN license in good standing; URAC-recognized certification in case management (CCM/CDMS/CRC/CRRN/COHN).
• Reliable transportation and ability to travel to in-person appointments.

Responsibilities

• Develop and implement treatment plans with physicians/providers, employers, and patients.
• Assess recovery and determine occupational vs non-occupational factors.
• Collaborate with physicians to tailor treatment plans for individuals.
• Work with employers to modify job duties to facilitate early return to work.
• Attend appointments and provide status updates to customers.
