Enlyte is seeking a Field Case Manager for a part-time, 30-hour week hybrid role. The candidate will travel locally for in-person appointments and coordinate treatment plans with treating physicians, employers, and patients.

The position emphasizes independent case management, strong communication, and clinical nursing expertise to achieve optimal recovery and return-to-work outcomes. Benefits and travel allowances are provided.

Qualifications

• Active RN license in good standing.
• 2+ years clinical practice preferred.
• Travel to in-person appointments within assigned geography.
• Proficient with Excel, Word, and Outlook Email.

Responsibilities

• Manage field case load and coordinate treatment plans with care teams.
• Collaborate with physicians, employers, customers, and patients to develop suitable plans.
• Arrange and attend in-person appointments and monitor patient recovery.
• Advise on cost-effective healthcare services and ensure appropriate care.
• Document progress and provide status updates to customers.
