Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess, plan, implement, coordinate, monitor, and evaluate care to meet members’ health and social needs, acting as a liaison among members, RP/POAs, PCPs, and Align Senior Care stakeholders.

You will collaborate with the care team and medical directors to ensure appropriate treatment, educate members and families, and support care transitions within a hybrid work environment.

Qualifications

• 1 year of clinical practice in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Proficiency with Word, Excel, Outlook and navigating multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Execute strategies to improve member experience.
• Contact members for health assessments and care plans.
• Educate members and caregivers about disease status, benefits, and resources.
• Coordinate with care teams to ensure appropriate treatment.
• Follow up with members to track progress toward goals.
