Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess member needs, plan and coordinate care, and serve as a liaison among Members, their RP/POAs, PCPs, and Align Senior Care stakeholders.

The role reports to the Supervisor of Case Management and supports hybrid field and home-office work. Travel to assigned facilities about 2–3 days per week, maintain thorough documentation in multiple systems, and help members access benefits, schedule

Qualifications

• One year of clinical practice experience in case management or related field.
• Proficiency using Microsoft Office and navigating multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Executes on strategies to manage and improve member experience.
• Conducts health assessments and develops plans of care with the interdisciplinary team.
• Educates members and families about disease status, benefits, and resources.
• Coordinates with care team and PCPs to facilitate appropriate treatments.
• Maintains timely communications with Members and RP/POAs and documents care plans.
