Job summary

The Care Ally, Case Manager will join an interdisciplinary team to assess, plan, implement, and monitor member health and social needs, coordinating with RP/POAs and the care team to ensure high-quality, person-centered care.

The role requires clinical experience in case management and strong collaboration skills, with hybrid work and travel to facilities.

Qualifications

• One year of clinical practice in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Proficiency using basic Microsoft Office (Word, Excel, Outlook) and navigating multiple systems.
• Case management certification preferred; long-term care or nursing facility experience preferred.

Responsibilities

• Executes on strategies and goals for managing and improving overall Member experience.
• Contacts Plan members to conduct health assessments, develop care plans, and participate in care team meetings.
• Serves as health coach to educate the member, family, and caregiver about disease status and plan benefits.
