Western Reserve Area Agency on Aging in Cleveland, OH is seeking an RN Case Manager to support long-term clients ages 60 and older enrolled in the MyCare Ohio CareSource waiver programs through clinical assessment, care coordination, advocacy, and ongoing case management.

Qualifications

• Current RN license in Ohio.
• Minimum of 1 year experience with individuals with disabilities in home health, medical social work, or behavioral health environments.
• Excellent communication, interviewing, and assessment skills.

Responsibilities

• Conduct comprehensive client assessments.
• Develop and implement individualized care plans.
• Coordinate home and community-based services.
• Advocate for client needs and quality care outcomes.
