Job summary

Western Reserve Area Agency on Aging (WRAAA) in Cleveland, OH, seeks an LSW Case Manager to support older adults and individuals with disabilities enrolled in MyCare Ohio CareSource waiver programs. You will perform clinical assessments, care coordination, advocacy, and ongoing case management across hybrid field-based settings.
