Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary team focused on improving health and dignity for older adults. You will assess needs, coordinate services, educate members and families, and collaborate with clinicians to ensure timely, appropriate care.

The role requires a valid RN or LPN license or a BSW with MSW preferred. A hybrid schedule, travel to facilities 2–3 days per week, and strong computer skills are expected.

Qualifications

• Active, unrestricted RN or LPN license in the state where job duties are performed; OR Bachelor’s degree in social work (BSW). MSW and licensure preferred but not required.
• Bachelor’s degree in social work (BSW); MSW preferred but not required.
• One year of clinical practice experience in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Experience coordinating care and using multiple computer systems.

Responsibilities

• Contacts plan members to conduct comprehensive health assessments and develop a plan of care.
• Educates members and families about disease status, plan benefits, and available resources.
• Collaborates with care team and medical directors to facilitate appropriate treatment.
• Addresses member grievances and utilization management issues.
• Schedules and coordinates appointments, monitors care plans, and communicates updates to the team.
