Job summary

Curana Health, Inc. seeks a Care Ally, Case Manager to join the interdisciplinary care team, coordinating health plans and services for seniors. You will engage with members, families, and care partners to implement care plans and monitor progress, acting as a liaison across departments.

You will educate members and families, coordinate with PCPs, and ensure timely access to services, while maintaining detailed documentation and patient-centered communication.

Qualifications

• One year of clinical practice in case management, home health, or related fields.
• Proficiency using Microsoft Office and navigation across multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.
• Active RN/LPN license or BS/W MSW; licensure preferred.

Responsibilities

• Executes care plans and coordinates services with ICT and care team.
• Contacts members to assess health needs and develop plans of care.
• Educates members and families about disease status, benefits and resources.
• Coordinates with PCPs and internal departments for appropriate treatment.
• Documents care team meetings and communicates updates to RP/POAs.
• Assists with scheduling, home health visits, and equipment coordination.
