Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary team. You will assess, plan, implement, coordinate, monitor, and evaluate services to support seniors and their families in a hybrid work arrangement.

You’ll coordinate with RP/POAs, plan providers, and internal teams while educating members about benefits and resources. The role requires traveling to facilities about 2–3 days per week and maintaining strong documentation and communication.

Qualificações

• One year of clinical practice in case management or related areas.
• Proficient with Word, Excel and Outlook; able to navigate multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.
• Active, unrestricted RN or LPN license in the practice state.
• BSW or MSW preferred; licensure may be required.

Responsabilidades

• Coordinate care as part of an interdisciplinary team to meet member needs.
• Conduct health assessments and develop plans of care.
• Educate members and families about disease status, plan benefits, and resources.
• Collaborate with care team and providers to facilitate appropriate treatment.
• Follow up with members to monitor progress and update care plans.
• Assist with scheduling, appointments, and home health/therapy visits.
