Job summary

The Center for Human Services is seeking a Service Coordination Supervisor in Independence, MO. The role oversees the SC Department, ensures documentation accuracy, and drives quality services for individuals served while coordinating with families, guardians, and community providers.

You will lead staff meetings, monitor performance, and promote a culture of communication and continuous improvement within a hybrid/remote work environment.

Qualifications

• Bachelor’s or Master’s in related field with required experience.
• Supervisory or lead experience preferred.
• Experience with case management documentation apps.

Responsibilities

• Oversees daily operations of Service Coordination Department.
• Supervises and develops SC staff, conducts evaluations.
• Recruits, interviews, hires, and trains team members.
• Ensures documentation accuracy and timeliness (TCM, ISP, reviews).
• Maintains effective communication with families and providers.
• Complies with DHMH statutes and CHS policies.
