Job summary

The Center for Human Services - Missouri is seeking a Service Coordinator to help individuals with developmental disabilities meet personal goals and maximize independence. You will support plans, monitor services, and advocate for clients in home, day programs, and community settings.

Responsibilities include building community relationships, coordinating resources, and documenting activities with timely case notes.

Qualifications

• Bachelor's degree or RN license is required.
• Must have reliable transportation and driving record; own vehicle for assignments.
• Valid driver’s license; Missouri license preferred but not required.
• Access to reliable internet for telecommuting.

Responsibilities

• Manage a caseload of varying ages and disabilities to improve independence and inclusion.
• Develop community links and coordinate resources to meet needs and create action plans.
• Monitor service quality, timeliness, and effectiveness via regular contacts and meetings.
• Document activities with timely case notes and maintain required monthly hours.
• Provide up-to-date information through training, learning, and conferences.
• Promote teamwork and take on special assignments to foster unity.
