Job summary

Qualifications

• Bachelor's degree from an accredited college or university.
• RN license is required.
• Plus four+ years in IDD-related community settings.

Responsibilities

• Manages a caseload to improve support quality and independence.
• Develops community links to meet client needs and plan actions.
• Monitors services, documents outcomes, and ensures QA processes.
• Maintains timely records and supports audits and training.
• Mentors Service Coordinators and coordinates with leadership.
