Qualifikationen

• Bachelor's degree and RN license required.
• Four or more years of professional experience with individuals with intellectual and/or developmental disabilities in community settings.
• Must live in the service area to be considered.

Aufgaben

• Manage a caseload of varying ages and needs to plan supports.
• Develop community resources and referrals to meet individual goals.
• Monitor and improve quality and timeliness of services through regular reviews.
• Maintain accurate documentation within three business days of activity.
• Mentor and train Service Coordinators on practices and regulations.
