Qualifications

• Bachelor's degree and RN license required.
• Four+ years of professional experience with individuals with developmental disabilities in community settings.
• Experience in case management and coordinating supports with multiple partners.
• Willingness to drive and travel as needed within service area.

Responsibilities

• Manage a diverse caseload to improve support quality and independence.
• Develop community linkages and coordinate resources and services.
• Monitor service effectiveness and ensure timely documentation and QA activities.
• Mentor and train Service Coordinators on best practices and regulatory requirements.
• Maintain compliance with reporting, audits, and program standards.
