Job summary

Qualifications

• Bachelor’s degree or RN license required.
• Reliable transportation and valid driver’s license.
• Experience working with individuals with developmental disabilities preferred.
• Ability to document and manage case notes accurately.

Responsibilities

• Manage a caseload across a range of ages and needs to support independence.
• Develop community connections and link resources to individual plans.
• Coordinate service authorizations and monitoring with providers and agencies.
• Maintain timely documentation and meet reporting deadlines.
