Qualifications

• MSW degree or equivalent advanced social services education; licensure or eligibility for licensure in DC preferred.
• Experience working with individuals and families experiencing homelessness, housing instability, or related social challenges.
• Knowledge of supportive housing models, local housing resources, and community-based social service systems.
• Ability to work collaboratively in a multidisciplinary environment and maintain professional boundaries and confidentiality.
• Proficiency in basic computer applications and electronic case management or client tracking systems.

Responsibilities

• Conduct assessments and develop service plans for Shelter Plus Care program participants.
• Monitor participant progress and maintain accurate documentation as required by program and funding sources.
• Coordinate with internal teams and external partners to access housing resources and community services.
• Facilitate referrals to mental health, substance use, and other supportive programs as needed.
• Track program outcomes and assist with reporting to support program development and continuous improvement.
