Catholic Charities of the Archdiocese of Washington is seeking a Shelter Case Manager to provide on-site case management, develop individualized goals, and support shelter clients in exiting to housing or employment. You will document progress within 48 hours, coordinate with program teams, and connect clients to benefits and housing resources.

Qualifications

• Associate's Degree or 2 years of college required.
• 2 years of experience with homeless service programs or social services required.
• Candidate may be waived if Peer Case Management Certification is held.

Responsabilités

• Interviews clients for direct service needs and develops goals with written short-term and long-term plans.
• Reevaluates goals with clients at regular intervals.
• Facilitates individual and group meetings and collaborates with multidisciplinary teams.
• Makes referrals for emotional, substance abuse, legal, medical, educational, vocational, employment, and housing needs.
• Creates and maintains case records, including assessments and service updates.
• Completes reports and maintains data in accordance with program standards.
