Job summary

Catholic Charities of the Archdiocese of Washington in Washington, DC seeks a Shelter Case Manager to provide onsite case management, develop individualized goals, document progress, and connect clients to housing resources and benefits. You will work with a multidisciplinary team to increase shelter exits.

This role requires an Associate’s degree or two years of college (or equivalent homeless service experience); certification waivers available for Peer Case Management certification.

Qualifications

• Associate’s Degree or 2 years of college required; homeless service or social services experience may substitute.
• Peer Case Management Certification waives the degree/experience requirements.

Responsibilities

• Interview clients for direct service needs and develop a written case plan with goals.
• Create and maintain case records, assessments, and service updates.
• Collaborate with multidisciplinary teams to address housing, employment, and health needs.
• Identify barriers to housing and employment and connect clients to resources.
• Complete reports and ensure documentation meets program standards.
