Job summary

Able Health Services Inc. is seeking a Coordinator of Community Services to join our team in a hybrid work location in Maryland. The role focuses on providing Targeted Case Management, advocacy, and resource coordination for the individuals we support.

You will collaborate with the person served and the team to develop and monitor a Person-Centered Plan, ensuring timely activity notes and seamless service delivery. Strong communication and problem-solving skills are essential.

Qualifications

• Bachelor’s degree in a human service field or related accreditation.
• Associate’s degree with 2 years’ experience in human services.
• 7 years’ experience in the human service field.
• Commitment to self-determination principles and client empowerment.
• Effective advocacy and relationship-building skills.
• Clear written and oral communication; familiarity with agency systems.

Responsibilities

• Coordinate activities to plan, access, and maintain supports desired by the individual.
• Advocate on behalf of individuals and assist them to advocate for themselves.
• Maximize participation of individuals in the team process and support leadership in meetings.
• Negotiate and resolve conflicts within the team.
• Develop and monitor Person-Centered Plans (PCP) per COMAR and policy; ensure implementation.
• Visit assigned individuals at least three times during the PCP year; support transitions as needed.
• Respond to incident reports promptly to safeguard health and safety.
• Document activities for billing and reporting per protocol.
• Identify and access alternative supports or funding to meet needs.
