24 Hour Home Care is seeking a Community Partnerships Manager to drive referral generation, admissions, and revenue growth through regional centers and community partners. This role reports to the Regional Manager and collaborates with operations to align referral activity with capacity and service delivery.

The position entails outbound sales, strategic outreach, and cross-functional collaboration across branches, with 3-5x monthly travel to the San Andreas Regional Center and weekly presence

Competenze

• 2+ years of experience in sales, business development, referral generation, or a related growth-focused role.
• Demonstrated success meeting or exceeding sales, referral, or performance targets.
• Spanish fluency required.
• Experience working with Regional Centers, healthcare, home care, or social services strongly preferred.
• Experience with field-based sales, relationship development, and CRM systems; Salesforce experience preferred.

Mansioni

• Develop and execute strategic sales and outreach plans to achieve referral, admissions, and revenue targets within assigned Regional Center territories.
• Build and strengthen relationships with Regional Centers and community partners through educational presentations, meetings, events, and consistent outreach.
• Identify and pursue new growth opportunities, including service line expansion, alternative referral channels, and community partnerships that support organizational objectives.
• Collaborate with branch Operations and leadership to align referral pipelines with staffing capacity, service readiness, and operational priorities.
• Track and analyze sales activity, referral performance, and return on investment using CRM tools, providing actionable insights and recommendations through regular reporting and Quarterly Business Reviews.
