Job summary

24 Hour Home Care in El Segundo, CA is seeking a Community Partnerships Manager to drive referrals, admissions, and revenue through strategic relationships with Regional Centers and community partners. The role develops and executes sales plans, builds partnerships, and identifies growth opportunities while representing the organization.

Hybrid position with in-office work 3x per week. Compensation combines guaranteed and variable pay, including a $10,200 car stipend, up to $12,000 quarterly

Qualifications

• Bachelor's degree in Business, Marketing, Healthcare Administration, or a related field.
• 2+ years of experience in sales, business development, referrals, or a growth-focused role.
• Proven track record of meeting or exceeding sales or performance goals.
• Spanish fluency preferred; experience working with Regional Centers, healthcare, home care, or social services strongly preferred.
• Experience using CRM systems, preferably Salesforce, with strong presentation and relationship-building skills.

Responsibilities

• Drive referral, admissions, and revenue growth by developing and executing strategic sales plans for assigned Regional Centers and community partners.
• Build and strengthen relationships through presentations, meetings, networking events, and ongoing outreach that increases brand awareness and referral activity.
• Identify and pursue new business opportunities, including service line expansion, community partnerships, and philanthropic initiatives that support organizational growth.
• Partner with Operations and leadership to align sales activity, pipeline management, and market opportunities with operational capacity and business objectives.
• Track, analyze, and report sales performance, referral trends, and pipeline activity while ensuring compliance with Regional Center, HIPAA, and regulatory requirements.
