Job summary

24 Hour Home Care in Oxnard, CA is seeking a Community Partnerships Manager to drive referral growth and revenue by building strategic partnerships with Regional Centers and community partners. You will develop sales plans, cultivate relationships, and expand opportunities to support organizational growth.

The role requires strong communication, a proven sales track record, CRM experience (Salesforce preferred), and the ability to travel.

Qualifications

Bachelor's degree in Business, Marketing, Healthcare Administration, or a related field preferred.
2+ years of experience in sales, business development, referrals, or a growth-focused role.
Spanish fluency preferred; experience working with Regional Centers, healthcare, home care, or social services strongly preferred.
Experience using CRM systems, preferably Salesforce, with strong presentation and relationship-building skills.

Responsibilities

Drive referral, admissions, and revenue growth by developing and executing strategic sales plans for assigned Regional Centers and community partners.
Build and strengthen relationships through presentations, meetings, networking events, and ongoing outreach that increases brand awareness and referral activity.
Identify and pursue new business opportunities, including service line expansion, community partnerships, and philanthropic initiatives that support organizational growth.
Partner with Operations and leadership to align sales activity, pipeline management, and market opportunities with operational capacity and business objectives.
Track, analyze, and report sales performance, referral trends, and pipeline activity while ensuring compliance with Regional Center, HIPAA, and regulatory requirements.
