Job summary

24Hr HomeCare, LLC is seeking a Community Partnerships Manager in Oxnard, CA to drive referral growth and expand admissions through strategic partnerships with Regional Centers and community partners.

You will develop and execute sales plans, foster relationships, and identify new opportunities while ensuring compliance and alignment with operations. Fluency in Spanish is required for this growth-focused role.

Qualifications

• Bachelor's degree preferred in business, marketing, healthcare administration, or related field.
• 2+ years in sales, business development, referrals, or growth-focused role.
• Proven track record of meeting or exceeding sales or performance goals.
• Spanish fluency required; experience with Regional Centers, healthcare, home care, or social services highly preferred.
• Experience using CRM systems, preferably Salesforce, with strong presentation and relationship-building skills.

Responsibilities

• Drive referral, admissions, and revenue growth with strategic plans for Regional Centers and partners.
• Build and strengthen relationships via presentations, meetings, and outreach to boost referrals.
• Identify new business opportunities including service line expansion and community partnerships.
• Partner with Operations to align sales activity and capacity with business objectives.
• Track and report sales performance, referral trends, and pipeline while ensuring compliance.
