Job summary

Trinity Health IHA Medical Group is seeking a Panel Coordinator to support outreach to patients with gaps in chronic disease management and preventive services. Hybrid work includes on-site days in Brooklyn & Clinton Primary Care and remote days from Central in Ann Arbor.

You will review eligibility, invite patients to services, monitor scheduling, and support quality initiatives using EMR systems and MyChart communications.

Qualifications

• High School Diploma or GED completed.
• Completion of approved health program such as MA.
• Onboarding completion for Panel Coordinator.
• Completion of CAN/PCT EMT program or equivalent.
• CMAC or similar health certs.

Responsibilities

• Review health plan eligibility lists and new patient registries.
• Invite patients to IHA for services via welcome package per protocol.
• Monitor scheduling of new patients.
• Collaborate with Quality team to identify patients who do not establish care.
• Provide systematic monitoring of chronic disease management and prevention registry.
• Use EMR tools and initiate patient outreach via MyChart messages, phone calls and mailings.
• Support quality improvement initiatives and disease management programs.
