Job summary

Trinity Health IHA Medical Group is seeking a compassionate and motivated Panel Coordinator to support outreach to patients with gaps in chronic disease management and preventive services. You will help promote the success of quality and population health programs and care coordination activities.

The role is hybrid with on-site days in Livonia and occasional work in Ann Arbor, plus remote days, offering day shift coverage and growth opportunities within a mission-driven organization serving

Qualifications

• Education: High School Diploma or GED; completion of approved health program such as MA program or Panel Coordinator Onboarding.
• 1-3 years healthcare experience preferred, or equivalent education/experience.

Responsibilities

• Review health plan eligibility and new patient registries.
• Invite patients to IHA via welcome package per protocol.
• Monitor scheduling of new patients.
• Identify patients who do not establish care with Quality team.
• Monitor chronic disease management and prevention registry.
• Use EMR tools; outreach via MyChart messages, calls, and mailings.
• Support quality improvement and disease management programs.
