Job summary

Trinity Health IHA Medical Group seeks a compassionate Panel Coordinator to support outreach to patients with gaps in chronic disease management. You will promote success of quality and population health programs and care coordination activities in a hybrid schedule in Southeast Michigan.

The role requires 1–3 years of healthcare experience and completion of accredited health programs, with onboarding and CMAC/AMCA certifications advantageous.

Qualifications

• High School Diploma or GED is required.
• Completion of an accredited health program such as MA is preferred.
• IHA Panel Coordinator onboarding program completion is required.
• Clinical Support Assistant/CAN/PCT EMT or equivalent program is preferred.
• Other approved certificate programs are acceptable.
• Certification by AMCA/CMAC is advantageous.
• 1–3 years of healthcare experience or equivalent education is preferred.

Responsibilities

• Review health plan eligibility lists and new patient registries.
• Invite patients to IHA services via a welcome package per protocol.
• Monitor scheduling of new patients.
• Collaborate with Quality to identify patients who do not establish care.
• Provide systematic monitoring of chronic disease management and prevention registries.
• Use EMR tools and initiate outreach via MyChart, phone calls and mailings.
• Support quality improvement and disease management programs.
