Job summary

Trinity Health IHA Medical Group in Michigan is seeking a Panel Coordinator to support outreach for patients with gaps in chronic disease management and preventive services. You will coordinate care and promote success of population health programs using EMR tools and patient communications.

This hybrid position requires a High School Diploma or GED and health program certificates, plus 1–3 years of healthcare experience.

Qualifications

• 1–3 years of healthcare experience or equivalent combination of education and experience.
• High School Diploma or GED required; completion of accredited/approved health program preferred.
• Successful completion of IHA onboarding programs and related certificates.

Responsibilities

• Review health plan eligibility lists and new patient registries.
• Invite patients to IHA for services per protocol.
• Monitor scheduling of new patients and report gaps in care.
• Collaborate with Quality team to identify patients who do not establish care.
• Monitor chronic disease management and prevention registries.
• Use EMR tools and outreach via MyChart messages, calls, and mailings.
• Support quality improvement initiatives and disease management programs.
