Job summary

Centene is seeking a care management specialist to assist in developing and coordinating holistic long-term care plans for members in Southern Cook County. This field-based, home-centered role involves about 50% local travel for visits, with remaining time spent working remotely.

You will evaluate needs, coordinate between members, families, and providers, identify resources, monitor plans, and educate on services and benefits.

Qualifications

• Experience in case management, preferably with HCBS waivers.
• Bachelor's degree and licensure criteria specified for Illinois Plans.
• LPN with one year experience in assessments is acceptable for Illinois eligibility.

Responsibilities

• Evaluate member needs and resources to recommend care plans.
• Develop long-term care plans and coordinate providers and community resources.
• Coordinate between member/family and care team to ensure service access.
• Provide resource support to members and families for employment, housing, etc.
• Monitor care plans, member status, and outcomes; adjust as needed.
• Interact with providers to ensure member needs are met.
• Collect and maintain member information for regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Educate members and families on procedures, options, and benefits.
• Provide feedback to leadership to improve quality and cost-effective care.
