Job summary

Centene is seeking a Care Management specialist to assist in developing holistic care plans for community-based members, including aging and physical disability waivers. The role is hybrid-remote with 50% travel to member homes and requires proximity to Huntley, IL.

Training runs weekdays, with ongoing scheduling from 8-5 CT to 8-4:30 CT with a 30-minute lunch. Ideal candidates have RN licensure in Illinois or a related bachelor/master’s degree and background in case management or advocacy.

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• RN license in Illinois or equivalent LPN with geriatric assessment experience.
• Preference for prior case management, advocacy or home visiting experience.

Responsibilities

• Assess member needs and coordinate care plans to meet outcomes.
• Develop long-term care plans and identify providers and resources.
• Coordinate between member/family and care team to ensure access to services.
• Provide education to members and families on procedures, options, and benefits.
• Monitor plans and outcomes and report to leadership for quality improvement.
• Perform on-site visits as needed and ensure regulatory compliance.
