Job summary

Centene Corporation is seeking a field-based Care Manager to support members across Southern Cook County with a primary focus on Oak Lawn and nearby towns. You will assess needs, develop long-term care plans, coordinate services, and educate members and families on available benefits while traveling to member homes.

This role requires about 50% local travel and a mix of remote work, with emphasis on HCBS waiver populations and community resource connections to improve health outcomes and quality

Qualifications

• Requires a Bachelor's degree and 1 year of related experience or equivalent experience.
• Experience with Home and Community-Based Services (HCBS) waiver populations preferred.
• Strong ability to coordinate care and connect members to community resources.

Responsibilities

• Evaluates member needs, resources, and recommends the best care plan for outcomes.
• Develops ongoing long-term care plans and identifies providers and community resources.
• Coordinates between members/families and care teams to ensure service access.
• Provides resource support for employment, housing, participant direction, independent living, and social needs.
• Monitors care plans and member status, offering recommendations based on needs.
