Centene is seeking a LTSS Service Care Coordinator in Oak Lawn, IL to develop, assess, and coordinate holistic care management for HCBS populations. This full-time role emphasizes case management experience and on-site visits to ensure access to services.

Responsibilities include evaluating member needs, coordinating with providers, and educating families on healthcare options. Flexible schedules with remote/hybrid/field/office options may be available, and competitive pay is offered.

Qualifications

• Bachelor's degree required
• 1 year of related experience or equivalent experience
• RN (Registered Nurse) - Illinois
• Bachelor's or Master's Degree in a human services related field
• LPN (Licensed Practical Nurse) with one year of experience in conducting assessments for the elderly

Responsabilités

• Evaluate the needs of the member and recommend/facilitate the best outcome plan.
• Assist with developing ongoing long-term care/service plans and identify necessary providers and resources.
• Coordinate between the member/family and care provider team to ensure service accessibility.
• Provide resource support for various member needs based on service assessment.
• Monitor care plans and member status, providing recommendations based on identified needs.
• Interact with long-term care healthcare providers to ensure member needs are met.
• Collect, document, and maintain member information to ensure compliance with regulations.
• Perform on-site visits to assess member needs and collaborate with providers.
• Facilitate education for members and families on healthcare options and benefits.
• Provide feedback to leadership on opportunities to improve care quality and service delivery.
