Job summary

• Experience with waiver populations and care coordination is preferred.

Responsibilities

• Evaluates the needs of the member, resources, and plans for best outcomes.
• Develops long-term care/service plans and identifies providers and community resources.
• Coordinates between member/family and care team to ensure services are accessible.
• Provides resource support for employment, housing, and independent living needs.
• Monitors plans, member status, and outcomes; provides recommendations.
