Job summary

Centene Corporation seeks a case management professional to coordinate holistic care management for aging and physically disabled members. The role is hybrid-remote with significant travel to member homes and requires residence within 30–40 minutes of Des Plaines, IL.

Training is standard M-F 8-5 CT with future hours 8-4:30 CT. Responsibilities include evaluating needs, developing long-term care plans, coordinating between families and providers, and educating members on services.

Qualifications

• Requires a Bachelor’s degree and 1 year of related experience.
• Preference given to applicants residing within 30-40 minutes of Des Plaines, IL (near Skokie, Niles, Morton Grove).
• Illinois Plan: RN licensed, or Bachelor/Master’s in human services, or LPN with experience.

Responsibilities

• Evaluate member needs and resources to recommend the best care outcome.
• Develop and update long-term care/service plans and identify providers and resources.
• Coordinate between member/family and care team to ensure services are accessible.
