Job summary

Centene is seeking a Care Manager to assist in developing, assessing, and coordinating holistic care management activities for aging and disability waivers. The role involves 50% travel to member homes and serves community-based members in the Huntley, IL area.

This hybrid-remote position starts with a Monday–Friday training schedule (8:00–17:00 CT) and shifts to a main schedule of 8:00–16:30 CT with a 30-minute lunch, offering comprehensive benefits and a mission-driven environment.

Qualifications

• Bachelor's degree and 1 year related experience.
• RN license in Illinois or Bachelor's/Master’s in human services.
• LPN with 1 year experience in assessments and elderly services.

Responsibilities

• Evaluates needs and resources to plan the best outcome.
• Develops ongoing long-term care plans and identifies providers.
• Coordinates between member/family and care team for service access.
• Provides resource support for housing, employment, and related needs.
• Monitors care plans and outcomes and offers recommendations.
• Interacts with long-term care providers to meet member needs.
• Collects and maintains long-term care member information for compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Educates members and families on procedures, options, and benefits.
• Provides feedback to leadership on quality and cost-effective delivery.
• Performs other duties and complies with policies.
