Job summary

Homeward Bound of Marin is seeking a Hybrid Resource Counseled Housing-Focused Case Manager in San Rafael, CA. You will support residents in housing programs for disabled adults, helping them maintain housing and build independence through onsite support and coordinated services.

The role emphasizes trauma-informed practice, Housing First principles, and data-driven progress tracking, with a mission to end homelessness and promote stability.

Qualifications

• Bachelor’s degree in social work, psychology, social welfare, or related field or two years of related residential/housing experience.
• Two years of experience serving under-resourced or homeless populations preferred.
• Experience in residential or housing programs and familiarity with Housing First, harm-reduction, and trauma-informed practices.
• Knowledge of SSI/SSDI and other entitlement programs; proficient with data entry systems (HMIS, Salesforce).
• Proficient in Microsoft Word, Excel, cloud-based tools, and Adobe; comfortable in diverse settings.

Responsibilities

• Maintain a hybrid caseload with regular case management and housing goal progress.
• Prepare and update care plans, notes, and progress reports in a timely manner.
• Coordinate services with internal teams and external partners for continuity of care.
• Ensure documentation compliance and meet funder requirements and timelines.
• Provide supportive, trauma-informed, client-centered housing stability services.
