HomeFirst is seeking an experienced Hybrid Case Manager to support the Emergency Interim Housing Via Del Oro program in San Jose. The role focuses on low-barrier, client-centered case management to transition residents to permanent housing.

You will develop housing plans, provide direct services, and collaborate with community partners to secure subsidies and resources. Strong organizational skills and compassionate engagement are essential.

Qualificações

• Minimum two years’ experience in social services; bachelor’s degree allowed in related fields.
• Lived experience or understanding of unhoused population.
• Proficient in Microsoft Office Suite.
• Experience serving unhoused and low-income populations preferred.
• Bi-lingual capabilities are a plus.

Responsabilidades

• Direct Service: manage caseload and develop individualized housing plans.
• Coordinate with partners to connect resources for participants.
• Document case activity in HMIS within 3 business days.
• Attend meetings and participate in case conferences; support housing goals.
• Maintain thorough records and ensure grant compliance.
