Job summary

You will coordinate services, conduct assessments, and help clients access benefits while maintaining detailed case records and reports for program oversight.

Qualifications

• Requires four-year degree in Social Work, Counseling, or Psychology or related field.
• Two to three years of experience with underserved populations; 5 years may substitute for a degree.
• Familiarity with Housing First and Harm Reduction concepts; Motivational Interviewing.
• Must have valid CA Class C driver's license and satisfactory driving record.

Responsibilities

• Coordinate services for chronically homeless individuals with complex needs.
• Provide case management, counseling, and advocacy within a multidisciplinary team.
• Develop comprehensive care plans, assess needs, and coordinate housing and healthcare services.
• Maintain case notes, reports, and data for program administration.
