Homeward Bound of Marin is seeking a compassionate Program Coordinator to support formerly unhoused families in permanent supportive housing through our Family Place program.

Reporting to the Senior Director of Family Programs, you’ll provide intensive case management, resource navigation, client advocacy, and housing support, coordinating with landlords, shelters, and community partners to promote housing stability and economic independence.

Formación

• Bachelor’s degree in social work, psychology, human services, or related field or equivalent experience
• Two to three years of experience with under-resourced populations
• Experience with case management, housing services, resource navigation, or community engagement
• Experience in residential or housing programs and Housing First knowledge
• Knowledge of tenant rights under fair housing laws

Responsabilidades

• Coordinate housing and supportive services for Family Place participants for 8-16 families
• Provide case management, housing retention and economic independence resources
• Document program eligibility per HUD requirements for chronic homelessness
• Coordinate housing placements with Coordinated Entry and local partners
• Maintain records, case notes, incident reports, home-visit reports, and reports
• Enter participant information in HMIS and Salesforce
• Provide crisis intervention and strengths-based problem-solving
• Support residents with life skills and money management
• Conduct habitability inspections and annual assessments
• Help fill housing vacancies by coordinating referrals
• Partner with program and volunteer managers for events
• Maintain confidentiality and professional communication
