AuthoraCare Collective seeks a CAP Case Manager to oversee care plans for CAP-C and CAP-DA recipients across Stokes and Rockingham Counties. The role requires monthly recipient visits, performance of home-based assessments, and collaboration with the Director of Kids Path to determine caseloads.

This full-time hybrid position works Monday–Friday, 8:00am–5:00pm, serving the local community with a focus on compassionate, personalized care. Travel within the service area is required.

Qualificações

• Bachelor's degree in social work or a related healthcare field.
• Two years of social work case management experience required.
• Pediatric case management experience preferred (2 years).
• Case management accreditation or certification preferred.
• Must be able to travel to homes within the service area.

Responsabilidades

• Directs and coordinates all CAP-C and CAP-DA services per individualized care plans.
• Use case management process to plan and prioritize care needs and interventions.
• Partner with RN case manager to complete annual recertification.
• Establish and maintain relationships with recipients/caregivers for ongoing needs assessment.
• Recognize cultural, ethical, and belief influences on decisions.
• Conduct home visits per NC Medicaid requirements.
• Provide telephonic support as needed to recipients and caregivers.
• Notify changes in care plans to family and stakeholders.
