Healthkeeperz, Inc. is seeking a CAP Case Manager to provide essential case management for the Community Alternatives Program for Disabled Adults and Children.

The role includes assessments, eligibility evaluation, and developing care plans to coordinate medical, social, and financial services for safe, independent living. The position supports hybrid work with regular home visits and phone check-ins, using the E-CAP system for documentation and compliance.

Qualifications

• Bachelor-level education with case management-related experience in health or home care settings.
• NC Medicaid certified training within 90 days where applicable.
• Ability to travel to office locations and patient homes as needed.

Responsibilities

• Conduct initial screenings and assessments of patients and families.
• Identify social service needs and connect to community resources.
• Develop and update individualized care plans; educate patients and families on options.
