Qualifications

• Bachelor’s degree in social work or related field.
• NC Medicaid-certified training within 90 days of employment.
• Experience in community health, case management or homecare preferred.

Responsibilities

• Perform initial assessments and develop person-centered care plans.
• Coordinate services with families, RNs, and interdisciplinary team.
• Monitor services with regular follow-ups and documentation.
