RHA Health Services, LLC is seeking a CAP Case Manager in the New Hanover & Brunswick area to coordinate care for beneficiaries. You will conduct assessments, develop care plans, and collaborate with RNs and an interdisciplinary team to support clients in the community.

The role emphasizes compliance with state guidelines, meticulous documentation, and ongoing case management to prevent institutionalization and improve quality of life for vulnerable populations.

Qualifications

• Strong understanding of Medicaid programs and compliance requirements.
• Excellent communication and interpersonal skills to work effectively with families and multidisciplinary teams.
• Ability to manage multiple priorities and maintain detailed records.
• Experience with medically fragile children or in pediatric healthcare settings and disabled adults with complex care needs.

Responsibilities

• Assessment and Care Planning: conduct assessments and educate about waiver services and community resources.
• Coordination of Services: link beneficiaries to services and collaborate with healthcare providers.
• Monitoring and Follow-Up: monthly calls, home visits, document progress and adjust plans.
• Counseling and Support: provide emotional support and assist families in navigating challenges.
• Documentation and Compliance: maintain records, ensure regulatory compliance and proper billing.
