Job summary

RHA Health Services, LLC is seeking a CAP Case Manager to provide critical case management for beneficiaries at risk of institutionalization, coordinating care with families, RNs, and an interdisciplinary team. The role emphasizes person-centered planning, service coordination, and strong documentation to ensure health and safety.

The position requires a social work or human services background, Medicaid training within 90 days, and experience with medically fragile children or complex care

Qualifications

• Bachelor’s degree in social work or equivalent field is required.
• NC Medicaid-certified training within 90 days of employment is required.
• Experience with medically fragile children or pediatric healthcare is preferred.

Responsibilities

• Assess beneficiaries and families to evaluate medical, psychosocial, and environmental needs.
• Develop individualized, person-centered care plans and coordinate services.
• Monitor services via calls and home visits; document progress and adjust plans as needed.
• Collaborate with RNs and interdisciplinary teams; advocate for beneficiaries and families.
• Ensure compliance with state regulations and proper billing/documentation practices.
